+Go Mobile App by Vivify Health is a bring-your-own-device (BYOD) remote patient monitoring solution that keeps patients connected to their care teams from virtually anywhere. Access to the app is limited to patients who are assigned to a program and invited by their healthcare provider.
Patients use the Vivify Health mobile app (available for iOS and Android) to follow personalized care pathways, share biometric data, and access tailored educational resources and can conduct video visits with their care team. In addition, patients using +Go Mobile App can send and receive secure in-app text messages.
Providers enroll patients either at the healthcare facility or by shipping Bluetooth-enabled remote monitoring devices directly to the patient’s home. Supported devices include blood pressure monitors, pulse oximeters, weight scales, glucose meters, pedometers, and thermometers, with additional devices available as needed.
The app connects to the compatible Bluetooth-enabled medical devices for automatic biometric data collection. However, biometric devices are optional; patients can manually enter their biometric readings, and depending on their program, biometrics may not be required at all.
Vivify Health's +Go Mobile App solution is flexible in content and experience and designed to help engage, empower, educate and shape patient behavior.
